Who Qualifies?

  • Enrolled in Medicaid
  • Lives in Missouri
  • Doctor confirms daily help is needed
  • Can direct and manage your care (or helper)
  • Caregiver passes a background check.
  • Not on Medicaid/waiver.
  • Lives outside of Missouri
  • No doctor’s note.
  • Can’t direct your own care
  • Caregiver fails background check
  • Under 18 years of age
  • Enrolled in Medicaid
  • Lives in Missouri
  • Doctor confirms help is needed with 2 plus daily tasks (dressing, bathing, eating)
  • Needs care due to age, illness, or disability
  • Live in your own home.
  • Not on Medicaid already
  • Live outside of Missouri or not in your own home
  • Under 18 years of age
  • No doctor’s note for needing help with 2 plus daily tasks
  • No serious illness/disability needing this level of care.

Medicaid Coverage          

Medicaid home care in Missouri is Provided through MO HealthNet for eligible residents.    

  • Be a Missouri resident and U.S. citizen or qualified non-citizen
  • Have a qualifying disability, chronic illness, or aging-related need
  • Meet MO HealthNet income and asset requirements
  • Have your care needs assessed through the state evaluation process.

 

Private Pay Home Care

Millennium Manor CDS also offers private Pay home care options in Missouri for those who do not use Medicaid.

  • No Medicaid plan required
  • No income or asset limits
  • Care begins after a simple needs assessment
  • Flexible payment methods accepted.
